持因私普通護照簽證申請原則上應由本人提出。持（外交、公務、普通因公）等因公護照，須經中國有關部委提出簽證申請。

1.一份申請表格，須認真填寫并貼一張近期照片。

三、意大利留學簽證面試指南
1.過往經驗
關于這一方面，簽證官會問到以前學習的學校、專業，工作的公司名稱等，以及相關的一些細節。針對這類問題，申請者只需如實回答，并適時主動的出示證明自己言論的相關文件，比如畢業證、成績單、辭職信、以前的名片等。簽證官還會問選擇意大利、目標學校、所學專業的原因，對于這部分問題，申請者要提高警惕。雖然有些申請者有畢業之后在美工作幾年或者移民的打算，但這正是我們重點向面試官回避的。
2.申請原因
我們可以從學術氛圍、可用資源、教育水平等方面強調意大利的優勢，尤其可以強調自己的學校在所學專業上的特點。當簽證官問申請者申請了幾所學校，分別是什么的時候，一般列舉6、7個相同檔次的即可。如果申請的學校過多或者檔次拉開的過大，可能會給簽證官一個為了到意大利而申請的印象，從而懷疑我們的留學動機。
